this S4M program is giving an error that - cant set a value of HKCR\skinner4moto\defaultIcon\(the system cannot find the file specified)
may because of i formatted my pc so that icon is missing. where to get that particular icon?

1. Soft buttons are the two soft buttons images, values and attributes that you can change;

As you can see here, its Main Menu on the left and iTunes on the right. You can move placement of the title, colors of the font, image file in .gif, etc.

2. Yes, it will work. But bare in mind the screen size of 176x220, do not go beyond.

3. What do you mean by affect? You can always play around with image files and you'll see the difference. See below;
